中文摘要:   目的:观察宁泌泰胶囊对雌性大鼠间质性膀胱炎的改善作用,结合组织炎性因子表达和血清激素水平的变化,探讨宁泌泰治疗间质性膀胱炎的作用和可能的机制。  方法:将40只成年雌性SD大鼠,随机分为空白对照组、模型组、宁泌泰低剂量组和宁泌泰高剂量组,每组10只。除空白对照组外,其余3组大鼠均采用硫酸鱼精蛋白膀胱灌注的方法进行间质性膀胱炎造模,于造模第16天起,宁泌泰低剂量组和高剂量组分别给予0.4 g·kg-1·d-1和1.0 g·kg-1·d-1宁泌泰悬液,模型组给予等量溶媒灌胃,连续14 d。对空白对照组不作干预。干预结束后观察各组大鼠膀胱壁形态学和炎性变化;电镜超微结构观察肥大细胞数量及脱颗粒变化;ELISA方法检测血清雌二醇(E2)和孕酮(P)水平和局部膀胱组织IL-1α、IL-1β、IL-6和TNF-α水平变化。  结果:与空白对照组比较,模型组大鼠膀胱壁增厚粗糙、不均一,血管变粗,并有少许炎性液体渗出,颜色变暗;宁泌泰高剂量和低剂量组较模型组膀胱壁均有不同程度恢复,且呈剂量依赖性。组织学和超微结构显示,宁泌泰高剂量和低剂量组较模型组炎性和肥大细胞数量减少,同时肥大细胞脱颗粒化亦相应减弱;宁泌泰高剂量组较模型组血清E2水平明显下降(P<0.05)。与空白对照组相比,模型组膀胱组织的IL-6和TNF-α水平均明显升高(P<0.05,P<0.01);与模型组比较,宁泌泰高剂量组IL-1β、IL-6和TNF-α水平均明显下降(P<0.05,P<0.01),宁泌泰低剂量组TNF-α水平显著降低(P<0.05)。  结论:宁泌泰胶囊对雌性大鼠间质性膀胱炎有明显改善作用,其作用机制可能与下调膀胱组织炎性因子IL-1β、IL-6、TNF-α和血清E2水平有关。
Abstract:  Objective:To observe the effects of Ningmitai Capsule on interstitial cystitis in female rats,and investigate the therapeutic effects and possible mechanism of Ningmitai Capsule on the interstitial cystitis combined with inflammatory factors expressions in tissue and serum hormones levels.   Methods:40 adult female SD rats were randomly divided into the blank control group,model group,Ningmitai groups with low- and high-dose,10 rats in each group. Except the blank control group,the model of interstitial cystitis was established by intravesical instillation of protamine sulfate in the other groups. From the 16th day of modeling,the Ningmitai groups with low- and high-dose were treated with suspension of Ningmitai Capsule at daily dose of 0.4 g/kg and 1.0 g/kg respectively,the model group was treated with equivalent solvent by intragastric administration,with a course of continuous 14 days. No treatment was performed in the blank control group. After intervention,the changes on the bladder wall morphology and inflammation were observed in each group. The amount and degranulation of mast cells were observed by electron microscopy. The serum levels of estradiol(E2) and progesterone(P) and the levels of IL-1α,IL-1β,IL-6 and TNF-α in local bladder tissue were detected by ELISA.   Results:Compared with the blank control group,the bladder wall of rats in the model group was thickened,rough and uneven,and the vessels became thicker,and there was a little inflammatory liquid exudation,and the color was darkened. Compared with the model group,the bladder wall of the Ningmitai groups with low- and high-dose was recovered to different degrees and in a dose-dependent manner. The histology and ultrastructure showed that,compared with the model group,the amounts of inflammatory cells and mast cells were decreased in the Ningmitai groups with low- and high-dose,and the degranulation of mast cells was also reduced. Compared with the model group,the serum E2 level was obviously decreased in the Ningmitai group with high-dose(P<0.05). Compared with the blank control group,the levels of IL-6 and TNF-α in bladder tissue were obviously increased in the model group(P<0.05,P<0.01). Compared with the model group,the levels of IL-1β,IL-6 and TNF-α were obviously decreased in the Ningmitai group with high-dose(P<0.05,P<0.01),and the level of TNF-α was significantly decreased in the Ningmitai group with low-dose(P<0.05).   Conclusion:Ningmitai Capsule can obviously improve the interstitial cystitis in female rats,and its mechanism may be related to the down-regulation of inflammatory cytokines(IL-1β,IL-6 and TNF-α)in bladder tissue and serum E2.
